Literature Review: Relationship of Chronic Disease Management Program (Prolanis) To Blood Pressure of Hypertension Patients

Prolanis is a program to improve the quality of life, health status, and optimal life expectancy. The called be good if it can status prolanis participants, activities are medical/educational consultations, home visits, reminder, gymnastics. This research aims explain relationship chronic disease management (prolanis) blood pressure hypertension patients. based on literature review with Scoping Review method. Research articles were obtained through Google Scholar DOAJ 10 results that match topic, which keywords found by 5 (50%) consultation articles, 4 (40%) reminder 8 (80%) exercise articles.
